After Your Baby is Born

Your first Child and Family Health visit is usually completed at your home (the Universal Health Home Visit - UHHV). Details can be found in your Personal Health Record (PHR or Blue Book) under the 1-4 week developmental check tab. Every woman in Northern Sydney Local Health District (NSLHD) receives their PHR while in hospital and is offered a UHHV within two weeks of their discharge from hospital.

This visit provides you and your family with the opportunity to talk about any concerns you may have with a Child and Family Health Nurse and to learn more about the Child and Family Health Service options in your local area.

The purpose of this visit is for the early identification of infant, parent and family concerns.

This visit will focus on:

  • Monitoring your baby’s health, growth and development
  • Support with infant feeding, including breastfeeding
  • Screening for family wellbeing and relationships
  • Providing appropriate parenting information, support and guidance
  • Providing information about relevant community services and supports


  • The Child and Family Health Nurse will listen to any concerns you have about yourself, baby or family. If you or your baby requires extra or further specialised help, the Child and Family Health Nurse can refer you or your baby to other health and community professionals or to other services to access help early.

    You can expect phone contact from the Child and Family Health Service within the first 2 weeks of being home with your baby.

    If you have not had this phone call in the first two weeks, or you have any concerns you would like to discuss earlier, please call your local Child and Family Health Centre.
